What is your financial policy?

Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money-order, MasterCard, Visa, Discover or American Express.

What insurance do you accept?

We participate with the following HMO and PPO organizations: Aetna, Anthem Blue Cross Blue Shield, Cigna, ConnectiCare, United Healthcare, Oxford and Multiplan. We also participate with most Workers Compensation PPO's, traditional Medicaid and Medicare as well as Medicare Advantage Plans provided by ConnectiCare (also known as VIP), Aetna, Cigna (also known as Access) and United/AARP (also known as Secure Horizons).

How do I get a refill on my prescription?

Whenever possible, prescription renewals should be called in two days before your supply is exhausted. Please have the telephone number of your pharmacy available. All prescription refills must be called in during working hours of 9:00AM to 4:00 PM. Due to the fact that our physicians cover for each other during nights and weekends, it is imperative that you have your prescriptions filled by your OAH physician during the normal business hours. Covering Physicians will not refill prescriptions without prior consent from your OAH surgeon.

What are your office hours?

We are open between 8:30AM to 4:30 PM on Monday through Friday. We have coverage 7 days a week 24 hours a day for emergencies.

What paperwork should I bring?

Be sure to bring a photo ID, your insurance card, referral if needed, and workers compensation authorization if needed.
